Target Audience
Specialties - Nursing, Pediatrics, Pharmacy, Social Work
Professions - Doctor of Nursing Practice, Genetic Counselor, Non-Physician, Nurse, Nurse practitioner student, Pharmacist, Pharmacy resident, Pharmacy Technician, Physician, Physician Assistant , Social Worker, Student

Overview

Sweat testing remains the ‘gold standard’ method to confirm the diagnosis of cystic fibrosis (CF) and is now compulsory for the timely and accurate assessment of newborns identified by CF newborn screening. With the recent update in the Clinical & Laboratory Standards Institute (CLSI) on sweat testing guidelines, C34 Sweat Testing: Specimen Collection and Quantitative Chloride Analysis, a review and examination of role of sweat testing in the diagnosis of CF seemed appropriate.

This course, composed of four modules, will review the technical performance of the test and address the history and clinical nuances in maintaining clinical testing consistency. Additionally, we will address the interpretation of the sweat testing results, including those patients in the indeterminate range as well as advances in CF diagnostic testing for those patients where sweat testing fails to confirm a diagnosis.

We have composed this course differently, to expand the audience to the entire CF care team, including CF center genetic counselors as well as laboratory directors and personnel. As we begin to appreciate the variety of presentations of CF, in those with residual or minimal function genetic mutations, we have tried to clearly present the role of sweat testing in the current CF diagnostic algorithm. Although this exercise is an advanced review of the topic, we hope that every member of the care team can learn more about sweat testing, its history and current role in diagnosing CF.

Objectives
At the conclusion of this activity, participants should be able to:

  1. Interpret the technical performance of the sweat test including consistent performance of the pilocarpine iontophoresis;
  2. Interpret the results and determine if and when a test needs to be repeated;
  3. Determine what options are available to help confirm or refute a diagnosis of CF including genetic analysis, nasal potential difference (NPD) and intestinal current measurement (ICM).
