Overview
The purpose of this conference is to provide a great list of topics and speakers regarding geriatrics and Age-friendly health systems; including review of key aspects of geriatrics and working together in teams to focus on Mentation, Medications, Mobility, and What Matters Most for geriatric patients along the health care continuum and the variety of health care systems. Topics will include older adults and medications, programs to support mobility in older adults, what matters most for caregivers, and the use of technology to support brain care.

Objectives
At the conclusion of this activity, participants will be able to: 

  1. Describe the 4Ms of the age-friendly framework;
  2. Describe older adults’ decision making preferences and relationship with medication management;
  3. Describe the non-traditional, community-based boxing training approach for the management of Parkinson disease and interpret evidence to date;
  4. Identify emerging technological opportunities for aging brain care and research;
  5. Explain what is measured by existing caregiving scales and the gaps that exist in the form of key items that are not currently measured;
  6. Describe at least 3 outpatient practice models with pharmacist integration; and
  7. Identify and describe primary roles of caregivers.
