Objectives
At the conclusion of this activity, participants should be able to:
  1. Describe the standard Healthcare Model and the status of community healthcare programs/services.
  2. Recognize aspects of a Health System realignment: Early Community Involvement with Innovative use of Community Health Workers.
  3. Analyze needs of community using strong integration of scientific discovery with implementation science.
